Prior to your appointment:
Prior to your appointment you should receive a reminder phone call, email or text message (you are not billed for the text), whichever method you choose. If this is your first visit you will be asked to provide some background information on your dental and medical history. Please also bring picture identification and your dental insurance information, if you have this type of coverage.
During your appointment:
Our qualified staff will assess your oral health and develop a personalized treatment plan that puts your specific needs first. During the first visit, we’ll gather general information, take digital x-rays and do an examination to identify your current dental conditions. With regard to x-rays, it is our policy to take our own set of digital x-rays for new patients. They help us diagnose problems not visible to the naked eye.
Infection controls and universal precautions protect clients and staff alike. Everyone benefits from rigorous infection control – you, your dentist, and the dental team. The cornerstone in a good and safe dental practice is the element of trust. Please feel free to discuss this topic with any team member of Konikoff Dentistry. Each team in our offices follows procedures recommended by several federal agencies: the Occupational Safety & Health Administration (OSHA), the Environmental Protection Agency (EPA) and the Centers for Disease Control (CDC).
Our yearly training and review includes measures, such as the following:
- Hand washing techniques with disinfectant soaps
- Gloves and face masks-personal protective equipment
- Chemical disinfection of countertops and surfaces
- Proper sterilization techniques
- Disposable materials
- First aid kit use
- Emergency Procedures
We sterilize all reusable dental equipment, including dental hand pieces. We use an autoclave device that kills bacteria and viruses by steam, heat and pressure. Testing on this equipment to ensure proper sterilization is performed on a monthly basis. The best defense against disease is information. The more you know about our daily procedures and policies, the more comfortable you will feel.
We pride ourselves in delivering the highest standard of care; therefore, complete diagnostic x-rays are necessary. We require a complete series of x-rays on new patients and patients who have not been to see us on a regular basis. If you have had this series done with another dentist in the past three years, we ask that you bring them with you to your appointment. If you do not have them or are not able to retrieve them from your prior dentist before your appointment with us, we will need to take x-rays for a proper assessment and bill you accordingly.

Please alert the office if you have a medical condition that may be of concern (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, steroid therapy etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, biophosonates etc…)

Alert to Parents
A parent or legal guardian must accompany all children under the age of 18 at the first visit and for any ensuing dental visits where treatment by the dentist will occur.In the interest of time please take a moment to complete these patient forms. Please bring them to your next appointment.
